DM #116: Make sure you don’t let the pendulum swing too far the other way! Yes, I am constantly and continuously preaching that where you think it is, it ain’t, but please don’t forget about the human being standing in front of you, in pain, asking for your help.
When I get on my soapbox I am preaching that a good clinician seeks to find the cause of dysfunction. But that same good clinician is also treating the patient, addressing their pain, and dare I say it, treating the symptoms!

With the advances in pain science, we are understanding more and more the role of the brain, and the ‘software’ when it comes to pain. However, we must be careful not to let the pendulum swing too far in the other direction and attribute all pain and treatment outcomes simply to the brain and the nervous system (aka the software). Sometimes we are in fact affecting the hardware, helping tissues to glide better past each other, affecting fluid dynamics, and changing pressure within that area. Additionally, treatment localized to the painful area can ABSOLUTELY help with pain management and mitigation as we tap into free nerve endings/interoceptors, alter cytokine presence, decompress an area, and, though possibly a bit antiquated in its explanation, affect change via the pain gate theory. Yes, it is through neurological means, but it still speaks to the role of treatment at the site of pain/dysfunction.

My point is simply that we must be careful to not get so caught up in the neuro approach that we forget the benefit to be had from and necessity of localized treatment.
