DM #274: Make the nervous system feel safe and watch your movement capabilities grow.
Posted a video earlier today demonstrating how a lack of motor control can present as decreased mobility. So, how to intervene? Reduce the postural demands, make the nervous system feel safe, then work on patterning that movement.
What do I mean by reduce the postural demands? Decrease the positional difficulties of movement by changing how gravity affects the movement. As long as you know what “muscles” are working and the direction that gravity is acting (hint: it doesn't change), you can modify ANY movement. By doing this you make the nervous system feel safe which means it doesn't feel the need to reflexively tighten up muscles and lock down joints to give you stability. Instead it says “oh, you safe, you got this”, and more readily grants you that motion.

So, when your patient has full passive mobility but can't actively perform the movement, consider changing the position in which you're having them perform that movement.
